Missed/Cancelled Appointments: We kindly request a minimum of 24 hours notice for cancellation of regularly scheduled with the exception of unforeseen emergencies or illnesses. Any regularly scheduled appointment cancelled less than 24 hours in advance will be charged a $80 fee, which will not likely be reimbursed by insurance.

Telephone communication requiring more than 30 minutes will be pro-rated at the hourly rate for standard sessions.  We typically do not discuss clinical matters via e-mail due to privacy and confidentiality concerns.

Written Materials: We require as much notice as possible when requesting written summaries, documentation, or letters. As mandated by law, we request that patients and families sign a release of information prior to my releasing any information to third parties. Documentation requiring more than 30 minutes to prepare will be pro-rated at the hourly rate for standard sessions.

Payment and Insurance: We accept medical assistance of Carefirst BCBS. For other insurance carriers, clients are responsible for service fees. Fees will be discussed at the time of our initial communication. I have a limited number of clients for which I provide services on a sliding scale. Sliding scale details can be discussed during your initial consultation.

A portion of the fee for services may be reimbursable through a client’s “out of network” insurance coverage. We will provide clients with a detailed receipt that can be provided to insurance companies in order to seek reimbursement as stipulated under your insurance plan coverage contract. The Family Village Support Center of Baltimore is not responsible for submitting insurance claims to out of network insurance carriers, nor are we responsible for communicating with your insurance company directly.
